Inconsistent form layouts have been a challenge for years, so this weekend we created a new layout and front end style for website forms.
The submit button used to be green with white text, which would not pass ADA requirements for high-contrasting colors. So the submit button is very dark with white text for ADA.
Radio Buttons and checkboxes have better styling in two-coumn form layouts.
The two Checkbox types (single choice affirmation, multiple selections) are now relabeled and located side by side in the form field type dropdown selector.

New "Date" and "Date + Time" form fields:
We added a date picker form filed type, so visitors don't have to enter a date (like a birthday) by hand.
We are also adding another "Date + Time" Field to record the time of date as a form entry as well (like choosing an appointment time).
